Conference Registration
Registration includes conference materials, continental breakfast, lunch, and refreshments on May 21, 2005 and continental breakfast on May 22, 2005. Cut-off date to register for conference meals is April 29, 2005.
REGISTRATION I (conference with meals): $80.00 After April 29, 2005 $90.00 On-site (May 20-22, 2005, limited) $105.00
REGISTRATION II – College/University Students (conference with meals): $50.00 After April 29, 2005 $60.00 On-site (May 20-22, 2005, limited) $75.00
On-line registration
will be closed on Wednesday, May 18. You can still register by fax or
phone.
College/University students include a copy of form indicating full-time enrollment during Spring 2005 academic term. If you are a full-time K-12 teacher and wish to attend the conference, please contact Julie Lamb <jlamb@niu.edu> directly.
Cancellations & Refunds: Requests for refunds must be made in writing prior to May 1, 2005. A full refund, minus a $15 service charge, will be issued for each request. No refunds will be made for requests postmarked after May 1, 2005.
